Clinics see the same pair every week. Similar hearing loss, a similar tone, and two completely different lives. One patient mentions it in passing. The other has not had a full night of sleep in months.

The 2018 clinical review in the New England Journal of Medicine puts it plainly: tinnitus is a perception generated in the brain's auditory pathways, and how much it bothers a person depends heavily on attention, emotional and stress systems.1 The NIDCD says the same thing in everyday language: for many people, the approaches that help most work on the reaction to the sound, not on the sound itself.2

2. EVIDENCEHow a quiet room makes it louder

Ask anyone who lives with it and the sequence repeats. The room goes quiet, the tone becomes easy to find, and finding it raises alertness. Alertness tells the brain this signal matters, so it keeps watch, and watching makes it louder. The loop runs hardest at bedtime.

Breaking it is less about the ear and more about the chemistry that turns alertness down. GABA is the brain's main inhibitory signal, the one that lets a system settle.

L-tyrosine is the raw material for dopamine and noradrenaline and has been studied for mental performance under stress.3 Choline, through acetylcholine, is involved in attention and memory, the very systems that decide what gets monitored.4

3. WHAT PEOPLE CUTDiet advice for tinnitus, checked against the evidence

The standard advice list for tinnitus is short and confident: no coffee, less salt, no wine. Two of those have been tested properly, and the results are not what the list implies.

A randomized, triple-blind, placebo-controlled trial compared caffeine with placebo in people living with tinnitus and found no meaningful difference in loudness or distress.7 A separate trial of abstinence concluded it was ineffective and potentially distressing, because withdrawal symptoms landed on people who were already sleeping badly.8

Salt, sugar and alcohol have been followed in large cohorts. The associations that turn up are modest and inconsistent, which is a long way from a food that drives the sound.9

Two trials, no meaningful effect on the tone. Stopping abruptly tends to cost more than it returns.

Little direct evidence in tinnitus. Its clearest effect is on sleep quality, which does track with how loud the night feels.

5. WHAT TO EXPECTWhat a US manufacturer built with them

A US manufacturer combined those six ingredients into a single under-the-tongue spray, on the reasoning that a spray taken in seconds each morning is easier to keep up than a handful of capsules. The company's presentation walks through each ingredient, the daily protocol, what a realistic two to three months look like, and a 90-day refund window that covers empty bottles.

It is worth saying what the presentation does not claim: the studies above tested ingredients, not the finished spray, and a share of users report no change. If the 11 p.m. loop is familiar, the presentation is a reasonable next twenty-five minutes.

6. LIMITATIONSSigns that need a professional first

Tinnitus that appears suddenly, in one ear only, with dizziness, hearing loss or a pulsing rhythm should be evaluated by a doctor or audiologist first.2 People who take medication for blood pressure, mood or Parkinson's disease should ask their doctor before using any product containing Mucuna pruriens. A dietary supplement is an addition to regular care, not a replacement for it.
